Summary:
Apex Therapeutics Services is looking for a Homecare Coordinator to manage and coordinate the delivery services to patients.
The Homecare Coordinator will work closely with healthcare providers, patients, and their families to ensure high-quality, compassionate care is provided in the home setting.
The ideal candidate should have a strong background in healthcare, excellent communication skills, and the ability to work independently.
Key
Responsibilities:
Collaborate with healthcare providers to develop and implement care plans for patients receiving homecare services Schedule homecare visits and communicate with patients and their families regarding the timing and frequency of visits Monitor patient progress and adjust care plans as needed Communicate with healthcare providers regarding changes in patients' conditions or treatment plans Educate patients and their families on homecare procedures and protocols Ensure all homecare services are provided in compliance with state and federal regulations Maintain accurate and up-to-date patient records Requirements:
